Please note:

1) For all INITIAL VISITS, please also book a Second Visit/Extended Follow Up 1 month after your initial visit to ensure we save a spot in our schedule & the appropriate amount of time and space to follow up with your concern.

2) For all TELEMEDICINE appointments you must be a resident of Ontario and located in Ontario at the time of the appointment. If you are travelling outside of the province, please reschedule your visit for when return.

3) For RETURNING PATIENTS: - If it has been less than 6 months since your last visit and you have completed your Initial and Second visits, please book a regular Follow Up Appointment. - If it has been more than 6 months since your last appointment, but less than 12 months, please book a Second or Extended Visit. - If it has been more than 12 months since your last appointment, please book a Reactivation Visit.
